Forms and quizzes

A form asks; a quiz asks and scores. Both end the same way, with a name, a phone number, and a recorded consent in your client list. Build them at Portal → Growth → Forms & Quizzes.

You never have to face a blank builder. Around 50 starter templates ship with the feature, grouped into quizzes, intake and booking, screening and eligibility, business and partners, client care, and mentorship onboarding. There are quiz templates built around topics like thyroid and hormones, fertility and cycle timing, and whether a home draw suits an older parent, plus intake forms for booking a draw and short forms for after the visit.

Every template demonstrates the same pattern: a handful of scored questions, a contact step, and a result that only appears once the person submits. Search the shelf, pick one, and edit, or start blank if you know exactly what you want.

A form is a set of steps, and each step holds blocks you drag in from the library: short and long text, email, phone, number, date, single and multiple choice, dropdown, address, a consent checkbox, a text block, an image, a file upload, a star rating, a divider, and locked content that stays hidden until submit. Buttons decide what happens next: go forward, go back, submit, submit and unlock, open a link, jump to your booking page, or open a phone or messages app on the visitor’s device.

For a quiz, each answer option carries a score and an optional tag. The Quiz results tab turns score ranges into outcomes: a title, an explanation, and a call to action. The tags travel with the lead, so you know what someone told you without opening the submission.

The style tab handles the look (presets, layouts, corner style, your logo and accent), so the page looks like your business.

Copy it or open it from the builder toolbar. The preview beside the editor renders exactly what a visitor sees, so you can test the whole flow before anyone else does.

A completed run creates a lead with the name, email, phone, and the consent box they actually ticked, plus the quiz score and tags. Every submission is kept with its answers so you can look back at what was said.
